Cascade risk is the risk that a single failure propagates through interconnected systems, triggering a chain of subsequent failures that amplifies far beyond the original event. The defining characteristic of cascade risk is that each failure creates the conditions for the next one — a vendor disruption triggers a supply delay, which triggers a production halt, which triggers a revenue miss, which triggers a covenant breach.
Every enterprise operates as a system of systems. Technology platforms connect to business processes. Business processes connect to vendor relationships. Vendor relationships connect to regulatory compliance. When you make a strategic decision — changing a platform, restructuring a team, entering a new market — you are pulling a thread in this interconnected fabric. Cascade risk is what happens when that thread is connected to more things than you realized.
Traditional risk management treats risks as independent items on a register. Cascade analysis treats risks as nodes in a network — where the interaction between risks is often more dangerous than any individual risk alone.
Why Traditional Risk Registers Miss Cascades

Risk registers catalog individual risks. Cascade analysis maps how risks interact. The most dangerous risks in any organization are rarely the ones at the top of the register — they're the ones that trigger chains nobody mapped.

The 2021 semiconductor shortage cascaded from chip fabrication through automotive manufacturing, consumer electronics, medical devices, and defense procurement — because dependency mapping at the industry level was incomplete. Dependency Mapping: The Foundation of Cascade Analysis
You cannot analyze cascades without first mapping what connects to what. Dependency mapping is the foundational stage of cascade analysis — and the stage most organizations skip.

What fails immediately when this fails? Direct connections. If your payment processor goes down, what stops working in the next hour? Which systems have no fallback? Which teams are blocked?
3-Step Analysis
What fails after the first failures? Secondary effects. The payment processor outage blocks order processing, which blocks fulfillment, which triggers SLA breaches with three enterprise customers whose contracts specify uptime guarantees.
5-Step Analysis
Where does the chain end — or does it amplify? Extended cascades that cross organizational boundaries. The SLA breaches trigger penalty clauses, which hit quarterly revenue, which affects the financial covenant on your credit facility, which constrains your ability to fund the infrastructure upgrade that would have prevented the original outage.
At each step, MAIA activates industry-specific cascade patterns from a library of 14 industry profiles. A cascade in financial services follows different propagation rules than a cascade in healthcare delivery or manufacturing. The industry library ensures the simulation reflects how things actually fail in your sector — not generic risk logic.
Real Pattern: Single Vendor Cascade
Consider a common scenario: a technology company decides to consolidate from three cloud providers to one. The risk register shows "vendor concentration risk" with medium likelihood and high impact. Standard mitigation: negotiate stronger SLAs.
Cascade analysis reveals the actual structure:
- Step 1: Single-provider outage disables the primary application stack, CI/CD pipeline, and customer-facing analytics dashboard simultaneously (shared infrastructure dependency)
- Step 2: Engineering team cannot deploy hotfixes during the outage because CI/CD runs on the same provider. Support team cannot access diagnostic tools. Customer escalations spike.
- Step 3: Two enterprise customers invoke force majeure clauses. The analytics dashboard outage means the marketing team cannot generate the campaign performance report due to the board tomorrow.

Designing Stabilizers Based on Cascade Paths
